Evanston Office

Dr. Frial's Evanston Office is located at 2530 Crawford Ave. Suite 206 Evanston, IL 60201 This is the central office and all inquires, billings, payments, and correspondences should be sent to this office. The office is just a few blocks east of the Westfield Shopping Center (formerly the Old Orchard Shopping Center), right off the Old Orchard Exit on I-94.  There is plenty of FREE PARKING on either side of the building. As you walk into the suite, please make yourself comfortable in the shared waiting area and I will meet you at your scheduled appointment time.  Please help yourself to water, tea, coffee and/or reading materials. Chicago - Streeterville Office

Dr. Frial's Chicago-Streeterville Office is located at 233 E. Erie St. Suite 409, Chicago, Illinois 60611  In the heart of Chicago's premier shopping and dinning, the office is accross the street from Northwestern Memorial Hospital and a few blocks from Northwestern Univeristy, the John Hancock Building, and Loyola University Chicago Campus.   It is very accessible by public transportation, just east and walking distance from the Chicago Avenue Red Line stop.  There is PARKING available in the building for $10 ($13/hour minus $3 discount with a validated ticket).  Please bring your parking ticket with you to your appointment and I will validate it for you.  From the garage, take the elevator to the 4th floor. As you walk out of the elevator, you will see a door to the right in front of the elevator (not next to the elevator).  Please make yourself comfortable in the shared waiting area and you will be greeted at your scheduled appointment time.  Please help yourself to the reading materials.
